If you prefer to email us, please send your name and phone number only and someone from our office will call you within one business day to schedule your appointment. For privacy reasons, please do not include any other patient-related information or diagnoses in your email.

Records to Send in Advance

To make your consultation as productive as possible, please arrange to send your records 48–72 hours before your appointment. Our team will coordinate record requests with you.

Imaging

  • CT scan with contrast (CD or digital)
  • MRI / MRCP if obtained
  • PET scan if performed
  • All radiology reports

Pathology & Labs

  • Biopsy or surgical pathology reports
  • Molecular / genomic profiling results
  • Relevant blood work (tumor markers, LFTs)

Clinical Records

  • Consultation notes from other providers
  • Prior operative reports
  • Endoscopy, ERCP, or EUS reports
